Unless you mean random diseases spawned, or diseases exclusive to surgery - If a doctor performs surgery without PPE while they have an infectious disease the patient is all but guaranteed to get it, no?

I like the concept, but I feel like too many people will flop over without knowing what has even happened to them with this. I can’t think of appropriately telling flavor messages for it when it’s happening
maybe creating standard operating procedures to MRP medical (such as putting anesthesia on patient)
Changes were made to surgery success chances a while ago to make anesthesia required for 100% success. Sterilizer has an actual impact since then as well, and doing operations on anything other than an operating table has substantial risks
If you mean making it a rule… we don’t want to require people to be competent at doing surgery before they’re allowed to be a doctor, as ass-backwards as that sounds. Sage is for learning roles as well. If you want lore-based SOP, go edit one into the wiki page

All i mean here, is not putting any certain diseases. Just in general if the doctor operating you didnt keep the standards. You would have a chance for your after-surgery wound to get infected, and in addition you would need a special cure or another surgery for it.
Also all the changes i am proposing here are already well working system. As i previously mentioned this is how medical works on Paradise, and i found it super interesting to play and simply enjoy it, thats why i thought describing it a bit might be good idea to get some inspiration from.
While generaly succeed chance sounds good. It doesnt have as much impact, while the damage from screwing up is easly recoverable.
Thats why infected wounds, would require player to put more attention into operating woth good standards as they would require another surgery or special medicine to cure them.
Else its about player mindset. Noone will lynch the doctor who dont use anesthesia/gloves/mask or operate with bloody dirty hands.
So maybe preparing special text with standard operation procedures to surgery or medical would change it a bit. (this is only targeted towards MRP)
My biggest problem is that it would be hard to make any reasonable change for MRP without affecting too much LRP, thats why internal bleeding would be a problem. On MRP you will come to medbay and they will cure you but on LRP everyone is doing everything by their own.
Anesthetic should be used too. Maybe if surgery is done while the patient is awake they scream and flail which causes there to be a chance for the surgery to fail? Same could be done with booze for ghetto surgery. I always try and use Anesthetic but people either don’t want it or think I’m gonna kidnap them.
How about graft/transplant rejections or implant infections/inflammations:
Just addition to make sure that donors and recipients blood matches to avoid rejection, but if that happens - treat it with immune suppressant drugs
Same can go with implants, that there is some sort of chance, that body will reject implant and it will require either surgery to remove, or some immune suppressant drugs/antibiotics.
I am suggesting this, because it is usually easier just to remove organ and replace it with healthy/cyber one rather than trying to fix it.
Small addons/changes could be:
Organs
Kidney system - bean shaped organs in pelvic region, they are not just to make urine, but they are also important for removal toxic waste and making some drugs work (for example mannitol IRL). In game, damage to this organ could reduce/impair the removal of drugs/chemicals from blood system. Dialysis machine could have a place here also. We could also have urgent medical condition - kidney stones. Not life threating, but causes to stop and lose conscience for couple of seconds due to severe pain.
Pancreas - high amount of alcohol consumption or overeating could give a small chance to cause inflammation, which would work like apendicitis.
Spleen - can be damaged due to severe brute damage and it causes blood loss(or if you are going to implement - internal bleeding). Removal surgery could fix it.
Urgent medical conditions
Stroke - trigger would be random one, same as heart attack, it could knock player unconscious/slow down and cause brain damage plus trauma, which could be fatal if untreated.
Anaphylactic shock or asthma - small amount of players at round start would get hidden trait that they are allergic to some sort of triggers (latex gloves, certain food and so on). If character consumes or touches these triggers, huge amount of histamine would appear in blood system or massive oxy loss.
Pneumothorax - air buildup around lungs, this could happen after severe brute damage to chest and without surgical intervention it could be fatal.
I would also like to see bone system, internal bleeding and wound infections due to lack of aseptic conditions. There could be also immune system which could interact with viruses, but i can’t figure out how to make it work.
